What are Transition Services?
IDEA 300.43 defines transition services as a coordinated set of activities for students with disabilities aimed at enhancing their academic and functional achievements for a successful transition from school to post-school activities like:
These services are customized to each student’s needs, strengths, and interests, including instruction, related services, community experiences, and the development of employment and daily living skills.
What is a Transition Plan?
Transition planning is a cyclical, ongoing process that may require stepping back to move forward. It extends beyond a single IEP meeting and involves continuous opportunities for students to explore their skills, interests, and abilities. Effective transition planning relies on collaboration with various individuals and agencies to define the necessary services for each student.
Key components of transition planning include:
