What are Transition Services?

Young adult with special needs learning to run a cash register and group of teens in a library.IDEA 300.43 defines transition services as a coordinated set of activities for students with disabilities aimed at enhancing their academic and functional achievements for a successful transition from school to post-school activities like:

  • Postsecondary education

  • Vocational training

  • Employment

  • Independent Living, when appropriate

These services are customized to each student’s needs, strengths, and interests, including instruction, related services, community experiences, and the development of employment and daily living skills.

What is a Transition Plan?

Transition planning is a cyclical, ongoing process that may require stepping back to move forward. It extends beyond a single IEP meeting and involves continuous opportunities for students to explore their skills, interests, and abilities. Effective transition planning relies on collaboration with various individuals and agencies to define the necessary services for each student.

Key components of transition planning include:

  • Assessing strengths, needs, preferences, and interests;

  • Identifying a vision for the future;

  • Creating and refining an IEP with measurable postsecondary goals and services;

  • Implementing the IEP and monitoring progress; and

  • Collaborating with others to ensure coordinated efforts.

These programs provide tailored support and resources to meet each student's unique needs, enhancing opportunities for success as they transition from school to adulthood.

Throughout the year, professional learning opportunities are available from the Genesee Intermediate School District and its partner providers.
